Vermilion NJCAA Physical Form

Athletic Worksheets are used by student-athletes during the first few days of their respective sports season.  Upon arrival, and through week one of the semester,  student athletes need to verify completed steps for admissions/enrollment, course registration, financial aid/bills and payments, and athletic forms. Student-athletes should meet with Vermilion Student Services staff to complete worksheet verifications and turn completed worksheets into the Ironhawks Lock Box in the Academic Resource Center cs107.  Go Ironhawks!
